SERVICES FOR PEOPLE WITH DISABILITIES...

Adult Day Program - Provides the opportunity to develop new skills and enhance independence through training in communication, money management, community inclusion and social skills plus the use of special adaptive equipment.

Supported Living - Provides the support necessary for participants to live in their own homes, assess "at risk" situations and develop safety nets and community resources. Staffing ranges from 24-hour coverage to "Drop-In" assistance.

Residential Homes - Homes modified to accommodate special needs and meet rigid safety requirements while maintaining a personalized, comfortable and well-designed environment.

Foster Care - Provides support to children in a home environment with a foster parent(s).

Supported Employment - Assistance for people with disabilities in getting and keeping jobs in the community.

Adult Social Groups - Provides opportunities for encounters and interactions in natural settings, such as games, outings and activities that include people with and without disabilities.

After-School Care - Provides meaningful, fun, safe and productive after-school hours at various Sunrise facilities for children and young adults with disabilities, who attend public school and live at home.

Respite Care - Enables families to take care of other responsibilities, by caring for children and adults with disabilities at day program sites or overnight in a residential setting.

Behavioral Analysis and Assessment - Assistance in changing problem behaviors; behavior analysis, developing, approving and implementing behavior service plans; monitoring procedures and outcomes plus training for parents, caregivers and staff.

Physical Therapy - When physical therapy services are prescribed by a physician, Sunrise therapists evaluate consumers and provide services necessary to produce specific functional outcomes in muscle control, ambulating and postural development to prevent or reduce further physical disability.

Nursing - Registered Nurses and/or Licensed Practical Nurses who provide those services which require a nursing license within that state, and to oversee the health needs of consumers.

Environmental Modifications - Physical adaptations are made to homes or work sites, which promote the health, welfare and safety of the individual, or enable an individual to function with greater independence.

Speech Language Therapy - Evaluations and services necessary to achieve specific, functional communication goals by individuals with speech, hearing or language disabilities.

Occupational Therapy - As prescribed by a physician, a program of activities or exercises to maximize the consumers' range of motion, oral motor skills and fine motor skills to achieve functional outcomes.

Companion Services - Assistance in hiring one or more companions to provide one-to-one, non-medical care, supervision and socialization opportunities for adults with disabilities in their own homes.

Infant Stimulation - Physical therapy, games, activities and interaction provided to children at an early stage, in a stimulating environment to enhance early childhood development.

In-Home Training - Support in general household activities, such as meal preparation and routine housekeeping, plus leisure and community integration provided by a homemaker.

Wheelchairs & Adaptive Equipment - Assistance to consumers in acquiring specialized equipment, tailored to their individual needs.

Transportation - Appropriate transportation for individuals who need or request it, primarily in vans, some of which are specially adapted.
